Terms of service.

By booking an appointment you agree to these terms of service.

1. You give full permission and consent to receive treatment, and understand what treatment will or can involve.

2. You understand that you will be required to expose the area to be treated, therefore please wear appropriate clothing to your appointment, e.g. sports bra for upper body issues and shorts if you have a lower body issue. Bra’s, shorts or underwear will not be removed under any circumstances and any area that is not being treated will be draped (covered with towels).

3. You understand the importance of accurately filling the patient form, including any relevant information of your medical history.

4. You understand that Mac Sports Therapy is not a substitute for medical treatment or medications.

5. You understand that Macaulay Burton - The therapist does not diagnose illnesses, injuries or prescribe medications.

6. You understand that it is your responsibility to inform the therapist - Macaulay Burton of any new symptoms, medications, injuries, discomfort before or during the session.

7. You understand that if you show any contraindications to treatment, the appointment will not go ahead.

8. You understand that if you are booking an appointment and are under 16 years old, you will need the signature of a parent or legal guardian and they will have attend the appointment to sign consent.

9. Cancellations within 12 hours will be charged 50% of the cost of the appointment. There is no charge for rescheduling. No shows with no prior contact will be charged 100% of the appointment cost.

10. You understand the late and cancellation policies. If you are going to be late, please contact at your earliest convenience, we will do our best to accommodate. If you are more than 15 minutes late, you will lose the appointment time, or your appointment will be cancelled unless contacted or agreed prior.
